You must complete this 2026 EAWC Client Intake Form – PDF (to download, print, and return to us) or this 2026 EAWC Client Intake Form – Online if you would like to have a volunteer attorney prepare your Will, Power of Attorney, and/or Advance Health Care Directive. Couples should each fill out and sign a separate intake form but may write “see spouse’s form” if some information is identical.
We suggest previewing the questions on the PDF form before completing the online form so that you can gather all information required before starting.

The Elder Appreciation and Wills Clinic is an annual clinic held during the Alaska Federation of Natives Convention in Anchorage in October each year. Volunteer attorneys draft wills, powers of attorney, and advance health care directives during two-hour appointments with low-income Alaskans. In addition, there will be drop-in resource tables with services and information relevant to Elders.
What to Expect
Appointments for Estate Planning Document Preparation:
Clients will have a two-hour appointment with a volunteer attorney, who will draft your Will, Power of Attorney, and Advance Health Care Directive. You must pre-register for an appointment and come prepared with your decisions made. See below for How to Attend the Clinic to pre-register.
Resource Tables
No appointment is required for the resource tables with services and information on topics relevant to Elders. We encourage you to drop by between 9 AM and 5 PM! There will be:
- Free memory screenings and dementia information (Alzheimer’s Resource Alaska)
- Aging and disability support (Aging & Disability Resource Center)
- Food pantry resources (Food Bank of Alaska)

Please drop by the Elizabeth Peratrovich Legal Clinic on October 22, 2026 from 11:30 AM to 2:30 PM to get 30 minutes of free advice from a volunteer lawyer. This clinic is also at the Dena’ina Center, Second Floor Room 5 in Anchorage. More details here.

How to Attend the Clinic

On Thursday, Oct. 22 from 11:30 – 2:30 PM in the same room (Dena’ina Center, 2nd Floor, Room 5, Anchorage), drop in and we can schedule your appointment for the Wills Clinic, space permitting.
Get an Appointment
We will contact you to schedule your appointment. If we have cancellations, we will try to accommodate walk-ins or, if you are eligible, we may refer you to Alaska Legal Services Corporation who will have intake staff at the clinic. We urge you to submit the intake form as soon as possible because we cannot guarantee that we can serve everyone. Make sure to check the email address you provided for the confirmation and add the phone number for the Alaska Bar Association – (907) 272-7469 – to your contacts.
Get Ready for Your Appointment
You should bring with you to your appointment:
- Your government-issued ID (like an Alaska driver license, passport, or birth certificate),
- Copies of deeds for all real estate and native allotments you own, including Title Status Reports and Inventories.
- A copy of your intake form (and review it to make sure it is complete and reflects your wishes), and
- Your current will, power of attorney, and advance health care directive, if you have them.
